1 Wash your hands. Dry them with a clean towel. Washing with warm water and rubbing your hands will help more blood to come to your fingertip.
2 Seat yourself so that you are comfortable. Be sure you have a clean, flat surface in front of you. Remove the TEST CARD from PANEL C. Place it nearby. Your blood sample will go on this card.
3 Detach the ID CARD from the TEST CARD. Put the ID CARD in a safe place. NOTE:You MUST have your personal identification number (on the ID CARD) to get your test result.
4 You will be taking a blood sample. Select either your index or middle finger. Choose a site to the side of your fingertip. Try to avoid calloused areas.
5 Remove one LANCET from PANEL B. Rub your fingertip with the ANTISEPTIC WIPE and let dry.
6 Hold the LANCET on your fingertip with moderate pressure. FIRMLY PUSH DOWN on the top of the LANCET with your thumb.
7 Press the base of your finger and push toward (but not all the way to) your fingertip. Repeat this until a large drop of blood is formed
8 Pick up the TEST CARD. Gently touch your drop of blood to the first circle on the front of the TEST card.Be sure to COMPLETELY fill the circle with blood until it soaks through to fill the circle on the back side.
9 Completely fill the second and the third circles with blood.
10 Make sure all three circles are filled with your blood.
11 Let the TEST CARD dry in the air for a few minutes. Do not touch the circles. Write the date you prepared your sample on the TEST CARD
12 IMPORTANT: NEVER reuse the LANCETS or share them with another person.
13 Remove the MAILER from panel ...... ENVELOPE ....... MAILER
14 Lift .... TEST CARD .... Make sure not to smear your blood sample.
15 Pull .... ENVELOPE
16 Lift ... MAILER
17 Pull ... expose the glue.
18 Fold ... Drop the MAILER into any mailbox. No postage is needed.

IMPORTANT : Avoid delay in mailing your sample..... Too much delay could have an adverse effect on your sample.
